5) The New Line Serial Number.
6) The new line number.
7) The amount you load last on
the old line.
8} Your email.
9) Your first & last name.
10) Your date of birth.
Thats all.
After getting all this set now visit

www.etisalat.com.ng/asimswap/
simswap.php
And
enter those information there.
Immediately after entering them
and you click
on the submit button etisalat will
send you a
confirmation message to your
email confirming
that they have seen your request.
in the confirmation will be:-
1) Time to expect your simswap done.
2) Review of the details that yu
entered.
But the sim swap/retrieval
shouldnt be more than
2days.
